New Patients

 Please note: Availability for initial evaluations is limited for the remainder of 2025.

For New Clients: We work on a referral basis. In order to schedule a new patient for an initial evaluation, we require a signed copy of our veterinary referral form. Please reach out to the veterinarian who has treated your pet for this issue and ask them to send us the signed referral form along with full medical records.

Most local veterinarians should have a blank copy of our referral form on hand. If your vet does not have the form, please have them contact us directly for a copy. 

Once we receive a signed referral, we will reach out to you directly to schedule an initial evaluation!
